If you are having surgery at Tamara Private Hospital, we will forward your completed Admission and Consent Forms to Tamara Private Hospital. You will be required to complete the Patient information form for Tamara Private Hospital either by completing the booklet or by accessing the Online Admissions at the top of our website.
